The Titan fits a niche that other trucks don't - it appeals to the younger generation. All three American makers are frankly making trucks that appeal to either working professionals (which is a small market) and people over 35. Ford outright says this about their F-series designs. Nissan appeals to the 30-under crowd. It's the reason their Frontier is such a good seller and the Ranger is gone. The niche the Titan fits, though, is a smaller market and without sells to pros, which requires capability - you're right - they'll never be a large market player.

Still, it's hard to beat Nissan's Frontier platform (which includes the Xterra) in off-the-lot offroad prowess without going to a more expensive Jeep or Rover. At both offroad events I attended and drove Jeep, Rover, and Nissan last fall, they were equal and the price tag on the Nissan is far lower. With stock vehicles from the three, I was able to do the same sand pits, rock crawls, brush grinding, etc. with only an adjustment to vehicle operation on my part (Jeep requires more "hands on" thought in gearing/throttle, Rover does everything automatically, and the Xterra falls between). Can't say the same for any of the GM or Ford SUVs.
